ABSTRACT

Eosinophilic meningitis (EoM) is an acute disease that affects the central nervous system. It is primarily caused by infection with the nematode Angiostrongylus cantonensis. This infection was previously restricted to certain Asian countries and the Pacific Islands, but it was first reported in Brazil in 2007. Since then, intermediate and definitive hosts infected with A. cantonensis have been identified within the urban areas of many states in Brazil, including those in the northern, northeastern, southeastern and southern regions. The goals of this review are to draw the attention of the medical community and health centres to the emergence of EoM in Brazil, to compile information about several aspects of the human infection and mode of transmission and to provide a short protocol of procedures for the diagnosis of this disease.

ABSTRACT

A 30-year old female presented with a one-month history of blurred vision in her left eye. Examination revealed a live motile worm in the anterior chamber of the left eye. She also had retinal pigment epithelial disturbance with focal intraretinal haemorrhage. The 19.9 mm worm was surgically removed and identified as Angiostrongylus cantonesis. She was treated with oral mebendazole. Her vision improved from counting fingers in the left eye to 6/36. This is the first documented case of ocular angiostrongyliasis in Jamaica.


Una mujer de 30 años se presentó con una historia de visión borrosa en el ojo izquierdo por un mes. El examen reveló la presencia de un gusano móvil vivo en la cámara anterior de su ojo izquierdo. También presentaba una alteración del epitelio pigmentario de la retina con hemorragia intraretiniana focal. El gusano de 19.9 mm fue extraído quirúrgicamente e identificado como Angiostrongylus cantonesis. La paciente fue tratada con mebendazole oral. Su visión mejoró - pasando de poder contar sólo sus dedos con el ojo izquierdo a una visión de 6/36. Se trata del primer caso de angiostrongyliasis ocular documentado en Jamaica.

ABSTRACT

Angiostrongylus costaricensis é um parasita que causa angiostrongilíase abdominal em humanos, seu tratamento inclui o uso de antiinflamatórios apesar da falta de estudos que justifiquem esta conduta. O objetivo deste artigo é avaliar o efeito da betametasona e da Arctium lappa na evolução de lesões intestinais induzidas pelo parasita. Utilizou-se camundongos Swiss, machos, adultos, distribuídos em 4 grupos: infectados tratados com betametasona; com Arctium lappa; não tratados e grupo controle. Os tratamentos iniciaram no 15º dia de infecção e permaneceram por 15 dias. Infiltrado eosinofílico e granuloma foram avaliados (1-leve; 2-moderado; 3-severo). A betametasona permitiu a evolução das lesões para formas mais graves, enquanto o extrato não interferiu na progressão da patologia. As substâncias empregadas não mostraram eficácia na proteção das lesões induzidas pelo Angiostrongylus costaricensis em camundongos. Estes achados desmotivam o uso de betametasona e Arctium lappa em humanos acometidos por angiostrongilíase abdominal.


Angiostrongylus costaricensis is a parasite that causes abdominal angiostrongyliasis in humans. The treatment for it includes the use of anti-inflammatory drugs, despite the lack of studies to justify this approach. The objective of this paper was to evaluate the effect of betamethasone and Arctium lappa on the evolution of intestinal lesions induced by this parasite. Adult male Swiss mice were used, distributed into four groups: infected and treated with betamethasone; infected and treated with Arctium lappa; infected and not treated; and control group. The treatments were started on the 15th day after infection and continued for 15 days. The presence of eosinophilic infiltration and granuloma was evaluated (1-mild; 2-moderate; 3-severe). Betamethasone allowed the lesions to evolve into more severe forms, while the extract did not interfere with disease progression. The substances applied were ineffective for protection against the lesions induced by Angiostrongylus costaricensis in mice. These findings discourage the use of betamethasone and Arctium lappa for humans affected by abdominal angiostrongyliasis.

ABSTRACT

Spigelia anthelmia Linn is used as a herb and is a common annual weed of cultivation in open re-growths, on unused land in towns as well as on road sides. The plant can grow to approximately 30 cm in height. The aim of this study was to screen extracts of Spigelia anthelmia for their anthelmintic activity against an experimental Nippostrongylus braziliensis infection in rats. Acute oral toxicity occurred at a dose of 1,140 mg/kg, while anthelmintic trials against Nippostrongylus braziliensis in rats using the aqueous fraction showed a progressive decrease in worm count with increasing dose (10, 13, 16, 20 and 25 mg per kg body weight) (p < 0.05). At 25 mg per kg body weight, the worm count was significantly lower than that at 10 mg per kg body weight (p < 0.05).

ABSTRACT

The clinical manifestations and outcome of patients with severe eosinophilic meningoencephalitis has never been reported. We reported 11 comatose patients with eosinophilic meningoencephalitis. Most of them presented with subacute to chronic headache and fever, followed by acute coma. Cerebrospinal fluid abnormalities were similar to alert patients with eosinophilic meningitis. None of them had received antihelmintic drug and seven patients were treated with corticosteroids. Ten patients died and one patient is still in a coma. Corticosteroids seem to be ineffective in severe eosinophilic meningitis.
